Choosing support involves questions about people, routines, safety and cost. Our intake process gives those questions a place before an agreement is signed. You can bring a family member, friend, advocate or support person to any stage.
We contact every new referral within three business days. That first conversation is a chance to explain what you need, how you would like to communicate and who you want involved.
This is our lived experience model in practice. Participant, parent, worker and management perspectives inform the process, while your own preferences shape the plan. Meeting your workers, understanding the agreement and being able to request change are practical parts of that approach.
Start with what matters to you
You can refer yourself, or someone can make a referral with your agreement. We ask about the support you want and complete a pre-risk assessment. This helps identify information, planning or worker skills needed to provide support safely.
Next comes a meet and greet at a time and place you choose. We discuss your goals, preferences, costs and rights. You can ask how the support would work in everyday situations, including the things you want to keep doing for yourself.
An informed decision on both sides
Both Directors consider whether Makodah Care can safely meet your needs. We explain the decision and the reasons. You also decide whether the proposed support feels right. If we cannot provide it, we explain why and discuss other options or a waiting list.
If you choose to proceed, we agree the supports, times, prices and responsibilities in writing. You can take time to read the service agreement and ask someone you trust to review it with you.
Your plan and your workers
Your support plan records your goals, routines, communication, health needs and agreed safety arrangements. You meet your support workers before supports begin. Your preferences matter in matching, and you can ask for a different worker at any time.
Once supports are underway, we review the plan with you at least every 12 months, at a new NDIS plan and when your circumstances change. You can request a review earlier. Feedback does not need to wait for a scheduled review.
