Your experience matters

Say what is working. Say what needs to change.

Give feedback or make a complaint to Makodah Care. Anonymous feedback is welcome, with acknowledgement within two business days and a written outcome.

Tell us in your own words.

Only the feedback type and message are required. You can review everything before taking the next step.

You can give a compliment, make a suggestion or raise a complaint. You can do this yourself or ask a family member, friend, advocate or support person to help.

Making a complaint will not affect your supports. You can leave your name out if you prefer, or choose how you would like us to contact you.

Our lived experience model depends on continuing to listen. Your experience of support is the reason to revisit an arrangement, explain a decision or address a concern. You can speak up, seek independent advice and ask for change.

Choose a way that works for you

Use the feedback and complaints form on this page, tell a worker or speak with our team. The Contact page lists direct contact options. Ask for an interpreter, Easy Read information or help from a support person if you need it.

Tell us what happened in your own words, when it happened if you know, and what you would like to happen next. You do not need to use formal language. Include the information you feel comfortable sharing.

What happens next

We acknowledge complaints within two business days, aim to resolve them within 21 days and provide the outcome in writing. If the matter will take longer, we tell you. The process includes listening to the people involved and considering the facts.

If you choose to remain anonymous and provide no contact details, we cannot send you an acknowledgement or outcome. You can still tell us about a concern. Our Participant Handbook explains the process, including independent consideration of a complaint about a Director.

You can go directly to the Commission

You can contact the NDIS Quality and Safeguards Commission at any time. You do not have to speak with Makodah Care first. An independent advocate can also help you understand options and express your views.

If someone is in immediate danger, contact emergency services. A feedback form is not monitored as an emergency response channel.
