An NDIS plan can bring several decisions at once: where to begin, which providers to consider and how the different parts will work together. Support coordination can help you make sense of those choices and take practical next steps.

Makodah Care offers support coordination with your priorities at the centre of the discussion. You may want assistance to set up new supports, consider a change or become more confident organising services yourself. Your preferences guide which questions need attention first.

Our lived experience model brings a participant perspective alongside professional coordination experience. We make room for the questions behind the plan: who you want involved, which providers you want to consider and how you want support to fit your week. The choices remain yours.

Turn the plan into useful questions

Start with what you want your support to make possible. Then consider what is already working, where the gaps are and which decisions need to be made. A clear list can help keep the conversation focused when several services or people are involved.

Support coordination may help you understand your plan, explore provider options and connect with relevant community or mainstream services. It can also help you build the skills to manage support arrangements. The work depends on your circumstances and the support coordination funding in your plan.

Choose providers with enough information

A useful provider conversation covers the actual service, suitability, availability, charges, communication and how changes are handled. You should have the opportunity to compare options and ask questions before agreeing. A familiar name or a quick answer is only part of that decision.

You can choose providers from different organisations. Using Makodah Care for support coordination does not require you to use our other services. Tell us about your existing relationships and any preferences you want considered when discussing options.

Keep your priorities visible

You may want to be involved in every call, prefer information in writing or ask a trusted person to attend a meeting. Discuss how you want communication to work and who you consent to involve. Support coordination should help you participate in decisions about your own support.

Our intake leads to an agreed service arrangement and support plan, followed by regular reviews. If circumstances change, use those conversations to revisit priorities and the next steps that are most useful. You retain the right to ask questions and choose a different provider.

Clear answers

A few things you might be wondering.

Is support coordination the same as plan management?

No. Support coordination helps you organise and use your supports. Plan management deals with the financial administration of plan funds, including paying providers. The roles are separate.

Do I need funding for support coordination?

Support coordination is a funded support that needs to be considered against your plan. Share the relevant plan information during enquiry so the proposed arrangement can be discussed accurately.

Can I keep my current support workers and providers?

Yes. Tell us what is working and which services you want to keep. You do not need to change providers simply because you are exploring support coordination.
