Referrals with the right information

A clear handover starts here.

Referral information for support coordinators, hospitals and other providers: consent, support needs, documents and Makodah Care’s intake process.

When you refer to Makodah Care, a clear picture of the person’s needs helps us assess whether we can provide suitable support. We contact every new referral within three business days.

We accept enquiries about our eleven service areas across Newcastle, Lake Macquarie, Port Stephens, the Hunter Valley and the Central Coast. Support arrangements and availability are assessed through intake before any service is agreed.

Our lived experience model brings participant and parent perspectives together with experience in support work, coordination and management. A useful referral includes the person’s own priorities and worker preferences alongside plans and assessment information, so those details can shape intake from the start.

Begin with consent and the person’s priorities

Confirm that the participant, or the appropriate guardian or nominee, agrees to the referral and to sharing the information included. Identify who should receive the first contact and how the participant prefers to communicate.

Include the person’s own priorities alongside the requested service. Useful context includes existing routines, worker preferences, cultural considerations and the role of family, carers or advocates. Where a formal decision-maker is involved, provide information about the scope of their authority.

Prepare the information that supports assessment

Tell us the location, requested support, proposed frequency and any timing considerations. Relevant parts of the NDIS plan, current support plans and existing risk or health plans help us assess the arrangements needed.

For complex needs, describe the support required and the current plans that guide it. Do not assume a service is confirmed because it appears on the website. Intake considers the specific needs, available workers and safe arrangements for that person.

  • Preferred contact and communication arrangements
  • Requested supports, location and timing
  • Relevant goals, support plans and health or safety information
  • Consent and the people authorised to participate in discussions

What happens after referral

The process includes first contact, a pre-risk assessment, a meet and greet and a decision by both Directors. We explain whether we can safely meet the person’s needs and the reasons for that decision.

If supports proceed, the service agreement and support plan set out the arrangements, and the participant meets their workers before supports begin. If we cannot provide the requested support, we discuss alternatives or a waiting list with the participant.
